Initial criteria
- The medication is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.
- Medication should not be used in patients with type 1 diabetes mellitus or diabetic ketoacidosis.
- Requesting agent must not be used in patients with a history of pancreatitis.
