  • Plan is Fully Insured or HIM Shop (SG)
  • No FDA labeled contraindications to Tarpeyo
  • The patient has another FDA labeled indication OR indication supported in compendia OR prescriber submitted two peer-reviewed journal articles supporting proposed use as generally safe and effective
