- Requested drug is NOT being used in a footbath
- Patient experienced an inadequate treatment response to a generic topical antifungal agent OR patient experienced an intolerance to a generic topical antifungal agent OR patient has a contraindication that would prohibit a trial of a generic topical antifungal agent
