Initial criteria
- Patient age ≥ 70 years
- Prescriber acknowledges that the medication is potentially inappropriate in older adults and that the benefit of therapy outweighs the potential risks for this patient
Reauthorization criteria
- Prescriber continues to acknowledge that benefits outweigh potential risks for the medication in the patient aged ≥ 70 years
Approval duration
12 months
