Streptomycin — CareFirst (Caremark)
Mycobacterium tuberculosis infection (used as part of a multi-drug regimen with INH, rifampin, and pyrazinamide or if other drugs are contraindicated)
Initial criteria
- Use for infections caused by susceptible strains only
- For tuberculosis, reserve as fourth drug unless resistance likelihood is low
- Reassess need for fourth drug once susceptibility results are available
- Seek expert consultation for complex resistance or HIV coinfection
