Streptomycin — CareFirst (Caremark)
Non-tuberculosis infections caused by bacteria shown to be susceptible to streptomycin and not amenable to therapy with less toxic agents including plague, tularemia, brucellosis, granuloma inguinale, chancroid, respiratory, endocardial, and meningeal infections due to H. influenzae, K. pneumoniae pneumonia, urinary tract infections due to E. coli, Proteus, Klebsiella aerogenes, K. pneumoniae, and Enterococcus faecalis, endocarditis due to Streptococcus viridans and Enterococcus faecalis, and gram-negative bacillary bacteremia
Initial criteria
- For tuberculosis, streptomycin should be used as part of a multi-drug regimen unless contraindicated
- For non-tuberculous infections, use only when infection is caused by susceptible bacteria and less toxic agents are not appropriate
