Tuberculosis as part of a combination regimen with isoniazid, rifampin, and pyrazinamide unless likelihood of resistance is very low

Initial criteria

For tuberculosis, streptomycin should be used as part of a multi-drug regimen unless contraindicated
For non-tuberculous infections, use only when infection is caused by susceptible bacteria and less toxic agents are not appropriate
