Pseudomembranous colitis or staphylococcal enterocolitis when administered orally using parenteral formulation
Initial criteria
- Patient has a proven or strongly suspected infection caused by susceptible bacteria
- Parenteral vancomycin should be used only when clinically indicated and oral therapy reserved for specific C. difficile or staphylococcal enterocolitis
- Culture and susceptibility testing should guide therapy
