Vancomycin Hydrochloride for InjectionCareFirst (Caremark)

Pseudomembranous colitis or staphylococcal enterocolitis when administered orally using parenteral formulation

Initial criteria

  • Patient has a proven or strongly suspected infection caused by susceptible bacteria
  • Parenteral vancomycin should be used only when clinically indicated and oral therapy reserved for specific C. difficile or staphylococcal enterocolitis
  • Culture and susceptibility testing should guide therapy
