Vfend IV — CareFirst (Caremark)

deep tissue Candida infections including disseminated infections in skin, abdomen, kidney, bladder wall, and wounds

Initial criteria

  • Patient is an adult or pediatric patient age ≥ 2 years
  • Diagnosis must be one of the indicated fungal infections
  • Specimens for fungal culture and other relevant laboratory studies (including histopathology) should be obtained prior to therapy to isolate and identify causative organism(s)
  • Therapy may be initiated empirically before results are known, but therapy must be adjusted once results become available based on susceptibility
