Initial criteria
- The requested drug is being prescribed for the management of moderate pruritus in an adult patient with atopic dermatitis or lichen simplex chronicus.
- The requested drug is being prescribed for short-term use (up to 8 days).
- The patient has experienced an inadequate treatment response to ANY of the following: a topical corticosteroid, topical tacrolimus ointment, Elidel (pimecrolimus), or Eucrisa (crisaborole).
