Initial criteria
- Patient is age ≥ 18 years
- Armodafinil/modafinil will be used in conjunction with positive airway pressure therapy OR patient is unable to initiate or tolerate positive airway pressure therapy
- If brand Provigil or Nuvigil is being requested, patient has tried generic modafinil or generic armodafinil AND brand product (Nuvigil or Provigil) is being requested due to formulation difference in inactive ingredient(s) which would result in a significant allergy or serious adverse reaction
