Initial criteria

• Patient was recently initiated on Chemet therapy in the hospital and further treatment is needed to finish the course of therapy; AND
• Medication is prescribed by or in consultation with a professional experienced in the use of chelation therapy (e.g., a medical toxicologist or a poison control center specialist)
