Chemet (succimer capsules − Lannett/Recordati Rare Diseases) — Cigna

Initial criteria

• Patient age between 12 months and 18 years; AND
• Prior to starting Chemet therapy, the patient’s blood lead level was > 45 mcg/dL [documentation required]; AND
• Chemet is being used for treatment of acute lead poisoning and not as prophylaxis against lead poisoning in a lead-containing environment; AND
• Medication is prescribed by or in consultation with a professional experienced in the use of chelation therapy (e.g., a medical toxicologist or a poison control center specialist)
