Initial criteria
- Patient age ≥ 18 years; AND
- Patient meets ONE of the following (i or ii):
- i. The medication is used in combination with a gonadotropin-releasing hormone (GnRH) analog; OR
- ii. Patient has had a bilateral orchiectomy.
