- Patient age ≥ 18 years AND
- Patient has tried at least one chemotherapy regimen (e.g., EPOCH-R, HyperCVAD, OFAR, RCHOP, or venetoclax + RCHOP) OR
- Patient is not a candidate for a chemotherapy regimen
