Initial criteria
- Patient is age ≥ 18 years
- Diagnosis confirmed by a sleep specialist physician or at a sleep disorders institution (sleep center)
- If brand Provigil or Nuvigil is being requested, patient has tried generic modafinil or generic armodafinil AND brand product (Nuvigil or Provigil) is being requested due to formulation difference in inactive ingredient(s) which would result in a significant allergy or serious adverse reaction
