• generic metformin immediate-release tablets (500 mg, 850 mg, and 1,000 mg strengths)

Initial criteria

• If the patient has tried one Step 1 immediate-release product, approve a Step 2 immediate-release product.
• If the patient is unable to swallow or has difficulty swallowing tablets containing metformin, approve metformin oral solution (Riomet, generic).
