Initial criteria

• Patient is currently receiving carbidopa/levodopa therapy; AND
• Patient has tried an entacapone product or Ongentys (opicapone capsules) and meets ONE of the following (i or ii):
• i. According to the prescriber, patient had significant intolerance; OR
• ii. According to the prescriber, patient had inadequate efficacy; AND
• The medication is prescribed by or in consultation with a neurologist.
