Initial criteria
- If the patient has tried one Step 1 Product, approve a Step 2 Product.
- If the patient has a known benzalkonium chloride (BAK) sensitivity, approve Timoptic in Ocudose (brand and generic) or Timoptic XE (brand and generic).
