Initial criteria

• age ≥ 18 years
• diagnosis of desmoid tumor (ICD-10: D48.11)
• desmoid tumor has progressed within the past 12 months
• patient requires systemic therapy for treatment of the desmoid tumor

Reauthorization criteria

• prescriber attests that the member has experienced positive clinical response to therapy

Approval duration

12 months
