Initial criteria
- age ≥ 18 years
- Patient is concurrently receiving other medication therapy for depression (e.g., SSRIs, SNRIs)
- If brand Provigil or Nuvigil is requested, BOTH of the following: patient has tried generic modafinil or generic armodafinil AND brand product is requested due to a formulation difference in inactive ingredient(s) causing significant allergy or serious adverse reaction
