Chemet (succimer) — Medica

Initial criteria

• Patient is between the age of 12 months and 18 years of age
• Prior to starting Chemet therapy, the patient’s blood lead level was > 45 mcg/dL [documentation required]
• Chemet is being used for treatment of acute lead poisoning and not as prophylaxis against lead poisoning in a lead-containing environment
• Medication is prescribed by or in consultation with a professional experienced in the use of chelation therapy (e.g., a medical toxicologist or a poison control center specialist)
