Initial criteria

• Patient has tried one Step 1 Product (generic ciprofloxacin 0.3% ophthalmic solution, generic gatifloxacin 0.5% ophthalmic solution, generic levofloxacin 0.5% ophthalmic solution, generic moxifloxacin hydrochloride 0.5% ophthalmic solution, or generic ofloxacin 0.3% ophthalmic solution) OR
• Patient has already started treatment with Besivance or Ciloxan ointment and requires approval to complete course of therapy
