Initial criteria
- age ≥ 18 years
- Armodafinil/modafinil will be used in conjunction with positive airway pressure therapy OR patient is unable to initiate or tolerate positive airway pressure therapy
- If brand Provigil or Nuvigil is requested, BOTH of the following: patient has tried generic modafinil or generic armodafinil AND brand product is requested due to a formulation difference in inactive ingredient(s) causing significant allergy or serious adverse reaction
