Viekira Pak — Medica

Initial criteria

• age ≥ 18 years
• Patient does not have cirrhosis OR patient has cirrhosis
• The medication is prescribed in combination with ribavirin
• The medication is prescribed by or in consultation with a gastroenterologist, hepatologist, infectious diseases physician, or a liver transplant physician

Reauthorization criteria

• Patient has been started on Viekira Pak and needs remaining duration to complete course of therapy

Approval duration

12 weeks without cirrhosis OR 24 weeks with cirrhosis
