Preferred products

• dextroamphetamine (Step 1 product for cataplexy in narcolepsy)

Initial criteria

• Patient is age ≥ 18 years
• Patient has been evaluated using polysomnography and a multiple sleep latency test
• Diagnosis of narcolepsy has been confirmed by the prescriber
• Medication is prescribed by or in consultation with a sleep specialist physician or a neurologist
• Patient has tried dextroamphetamine OR has a contraindication or intolerance to dextroamphetamine
• Do not use concomitantly with an oxybate product or Sunosi (solriamfetol)

Reauthorization criteria

• Diagnosis of narcolepsy with cataplexy remains confirmed and treatment continues to be prescribed by or in consultation with a sleep specialist or neurologist
