Chronic antibiotic-dependent pouchitis

Initial criteria

  • patient has recurrent pouchitis (typically ≥ 3 episodes within 12 months)
  • episodes respond to antibiotic therapy but relapse shortly after discontinuation
  • alternative causes of recurrent pouchitis have been ruled out
  • patient has tried long-term antibiotic therapy (at least 4 weeks) with BOTH ciprofloxacin and metronidazole for remission maintenance
  • prescribed by or in consultation with a gastroenterologist
