Initial criteria

• age ≥ 18 years
• has engaged in trial of behavioral modification and dietary restriction for ≥ 3 months
• at baseline had BMI ≥ 30 kg/m2 OR BMI ≥ 27 kg/m2 with ≥ 1 weight-related comorbidity (hypertension, type 2 diabetes, dyslipidemia, obstructive sleep apnea, cardiovascular disease, knee osteoarthritis, asthma, COPD, metabolic dysfunction-associated steatotic liver disease/non-alcoholic fatty liver disease, polycystic ovarian syndrome, or coronary artery disease)
• used concomitantly with behavioral modification and reduced-calorie diet

Reauthorization criteria

• age ≥ 18 years
• baseline BMI ≥ 30 kg/m2 OR BMI ≥ 27 kg/m2 with ≥ 1 weight-related comorbidity (as listed)
• has lost ≥ 5% of baseline body weight
• used concomitantly with behavioral modification and reduced-calorie diet
