Initial criteria

• age ≥ 18 years (adult criteria implied)
• patient weighs ≥ 60 kg
• Zontivity is prescribed in combination with aspirin and/or clopidogrel
• prescriber determines patient is at high risk for future thrombotic events (e.g., multiple myocardial infarctions, multiple urgent coronary revascularization procedures, placement of coronary artery stents, or other concomitant diseases such as diabetes)
