Initial criteria
- Patient age ≥ 18 years; AND
- Patient received ustekinumab IV induction within 2 months of starting SC; AND
- Patient either: trialed one systemic UC agent OR patient has pouchitis AND tried antibiotic, probiotic, corticosteroid enema, or mesalamine enema; AND
- Prescribed by or in consultation with gastroenterologist
Reauthorization criteria
- Patient established on ustekinumab SC ≥6 months; AND
- Patient had beneficial response assessed by objective measure (e.g., biomarkers, endoscopy, imaging) OR symptom improvement (pain, fatigue, stool frequency, bleeding)
Approval duration
initial 6 months, reauth 1 year
