Initial criteria
- The patient has a documented diagnosis of chronic pain or end-of-life pain requiring around-the-clock, long-term opioid treatment
- Alternative treatment options are inadequate (non-opioid analgesics and immediate release opioids)
Approval duration
6 months (12 months if sickle-cell, cancer-related, or end-of-life pain)
