seizures associated with tuberous sclerosis complex (TSC)

Initial criteria

  • Diagnosis of seizures associated with Lennox-Gastaut syndrome (LGS) AND patient age ≥ 1 year AND trial and failure with TWO anticonvulsants used for LGS (e.g., clobazam [Onfi], lamotrigine, valproate, rufinamide [Banzel], felbamate [Felbatol], levetiracetam [Keppra]) AND prescribed by or in consultation with a neurologist
  • Diagnosis of seizures associated with Dravet syndrome (DS) AND patient age ≥ 1 year AND prescribed by or in consultation with a neurologist
  • Diagnosis of seizures associated with tuberous sclerosis complex (TSC) AND patient age ≥ 1 year AND prescribed by or in consultation with a neurologist
