Cystic fibrosis

Initial criteria

  • Documented diagnosis of cystic fibrosis
  • Prescribed by or in consultation with a pulmonologist or physician specializing in the treatment of cystic fibrosis
  • Patient age ≥ 1 month
  • Documentation of a cystic fibrosis mutation test confirming one of the CFTR gene mutations responsive to Kalydeco
