Kerendia (finerenone) — Point32Health

Initial criteria

• Patient age ≥ 18 years
• Patient has heart failure with left ventricular ejection fraction (LVEF) ≥ 40%
• Patient has had trial and failure, inadequate response, intolerance, or contraindication to both spironolactone AND eplerenone
• Patient is currently taking, or has had trial and failure, inadequate response, intolerance, or contraindication to a sodium-glucose cotransporter 2 (SGLT2) inhibitor (e.g., Farxiga, Jardiance)
