active lupus nephritis in adults with systemic lupus erythematosus

Initial criteria

  • Documented diagnosis of systemic lupus erythematosus
  • Documentation of presence of lupus nephritis confirmed by urine/blood tests or kidney biopsy
  • Prescribed by or in consultation with a rheumatologist or nephrologist
  • Patient age ≥ 18 years
  • Documentation of one (1) of the following: (a) Use in combination with mycophenolate and a systemic corticosteroid OR (b) Clinical inappropriateness of mycophenolate and systemic corticosteroid
  • Documentation of one (1) of the following: (a) Inadequate response to Benlysta OR (b) Clinical inappropriateness of Benlysta
