Initial criteria
- Patient has had a trial of two Step-1 medications (lubiprostone, Linzess) within the previous 999 days as evidenced by a previous paid claim under the prescription benefit administered by the plan OR documented use by physician, excluding the use of samples
- OR patient has contraindication or intolerance to all Step-1 medications
