Omnipod 5 Intro Kit — Point32Health

Initial criteria

• One of the following: patient has a diagnosis of type 1 diabetes mellitus and is age ≥ 2 years OR patient has a diagnosis of type 2 diabetes and is age ≥ 18 years AND
• Patient is currently using a Dexcom G6 or G7 or Freestyle Libre 2 Plus Sensor continuous glucose monitor OR
• Patient has a diagnosis of type 1 diabetes mellitus and is age ≥ 2 years OR patient has a diagnosis of type 2 diabetes and is age ≥ 18 years AND member requires insulin injections at least 3 times per day or use of an insulin pump AND member has been recommended to perform frequent home blood glucose monitoring (≥ 4 times per day)
