Cystic fibrosis

Initial criteria

  • Documented diagnosis of cystic fibrosis
  • Prescribed by or in consultation with a pulmonologist or physician specializing in the treatment of cystic fibrosis
  • Patient age ≥ 1 year
  • Documentation confirming the patient is homozygous for the Phe508del (F508del) mutation in the CFTR gene
