neurotrophic keratitis, stage 2 or 3

Initial criteria

  • Documented diagnosis of stage 2 or 3 neurotrophic keratitis
  • Prescribed by an ophthalmologist
  • Documented trial and failure with one (1) conventional non-surgical treatment for neurotrophic keratitis (e.g., preservative-free artificial tears, gels, or ointments; discontinuation of preserved topical drops and medications that can decrease corneal sensitivity; therapeutic contact lenses)

Reauthorization criteria

  • Additional treatment beyond 8-weeks will not be authorized

Approval duration

8 weeks (one-time course per affected eye)
