Siliq (brodalumab) — Point32Health

Initial criteria

• Documented diagnosis of plaque psoriasis
• Patient age ≥ 6 years
• Prescribed by or in consultation with a dermatologist
• Documentation of ONE of the following:
• Trial and failure of at least two medications from EACH of the following therapeutic categories (one medication required if only one is available): Interleukin Antagonists (Bimzelx, Skyrizi, Taltz, Tremfya, Yesintek); Phosphodiesterase 4 inhibitors (Otezla); Tumor Necrosis Factors (Cimzia, Enbrel, Humira)
• OR Contraindication to ALL of the following medications: Bimzelx, Skyrizi, Taltz, Tremfya, Yesintek, Otezla, Cimzia, Enbrel, Humira
