Cystic fibrosis

Initial criteria

  • Documented diagnosis of cystic fibrosis
  • Prescribed by or in consultation with a pulmonologist or physician specializing in the treatment of cystic fibrosis
  • Patient age ≥ 2 years
  • Documentation of one of the following: (a) patient has at least one Phe508del (F508del) mutation in the CFTR gene OR (b) patient has at least one mutation in the CFTR gene that is responsive to Trikafta
