Initial criteria

• Diagnosis of chronic heart failure OR diagnosis of chronic kidney disease (CKD)
• Submission of medical records confirming that the patient is established on background loop diuretic therapy (e.g., bumetanide, furosemide, torsemide) for the treatment of edema
• Patient does not require ongoing emergency care or hospitalization
• Patient is currently a candidate for parenteral diuresis outside of the hospital
• Furoscix is prescribed by or in consultation with a cardiologist OR nephrologist
