Initial criteria

• Diagnosis of chronic kidney disease (CKD) associated with type 2 diabetes (T2D)
• Urinary albumin-to-creatinine ratio (UACR) ≥ 30 mg/g
• eGFR ≥ 25 mL/min/1.73 m2
• Used to reduce the risk of any of the following: sustained eGFR decline OR end-stage kidney disease OR cardiovascular death OR non-fatal myocardial infarction OR hospitalization for heart failure

Reauthorization criteria

• Documentation of positive clinical response to therapy
