Initial criteria

  • Zymfentra is not medically necessary for treatment of ulcerative colitis; all requests for authorization will be denied.
  • Zymfentra is not medically necessary for treatment of Crohn’s disease; all requests for authorization will be denied.

Reauthorization criteria

  • All requests for authorization (including reauthorization) will be denied as the medication is excluded from coverage for the majority of benefits.
